Tiny mistake: S2 episodes from 5 to 12 are mistitled by the titles of their counterpart episodes from S1.
However, I can see that the sizes are different, so it's probably just a problem with titles not the episodes themselves.
And I tried episode 9 from s2, and it's not episode 9 from s1, thankfully.

TL;DR: S2 titles (ep 5->12) are wrong, but the episodes themselves probably have no problems.
I tested one episode (9) and found it indeed S2 EP9 despite the wrong title, and I will confirm again when I watch the rest.
EDIT: Episode 10 from S2 is also the correct episode despite the wrong title...

This patch will fix the filenames of S2 episodes from 5 to 12.
Just but the .bat file in the same folder of S2 episodes, run it, done!
I grabbed the correct titles from IMDB.

In S2, Ep 25 from the Intro 00:00:00 to OP 00:00:18 the subtitles for the dialogue are missing. I've corrected the subtitle file based on the dialogue in Ember's webrip version. Why is the signs subs always the default? it so tedious having to change it every episode. so I have to mkvtoolfix away all subs but the one I want for every single episode.
since this happen in every anime i download that have both signs and dialog subs i am guessing it is something I do not understand. So i am posting this more to learn than to complain.

If you run linux or a mac you can use the command line tools to mass change all the .mkv files in a directory. It saves a LOT of time. Just cd into the directory with the videos you want to modify and run the command. Here are some useful examples, just swap out the variables (v1 = first video track, a2 = second audio track, s3= third sub track, etc):
Modify first video track to jpn language
find ./ -name "*.mkv" -exec mkvpropedit {} --edit track:v1 --set language=jpn \;
Modify first audio track name
find ./ -name "*.mkv" -exec mkvpropedit {} --edit track:a1 --set name="name" \;
Set Title
find ./ -name "*.mkv" -exec mkvpropedit {} --edit info --set title="The movie" \;
Unset Forced flag on subtitles
find ./ -name "*.mkv" -exec mkvpropedit {} --edit track:s1 --set flag-forced=0 \;
Unset Default flag on subtitles
find ./ -name "*.mkv" -exec mkvpropedit {} --edit track:s1 --set flag-default=0 \;
Mass add attachments
find ./ -name "*.mkv" -exec mkvpropedit {} --add-attachment <file.name> \;
You might find a way to do this in Windows Powershell too, but I'm not a proficient with PS.
